Injertos autologos para reconstrucción de reborde alveolar

Abstract

Actualmente existen diversos injertos óseos para reconstruir la estructura ósea del reborde, que pueden ser del tipo autólogo, homólogo, heterólogo y de origen artificial, los que tienen un papel importante dependiendo del tejido del cual sean extraídos, por lo que el de mayor compatibilidad y con mejores resultados será el injerto autólogo, llamado así porque se obtiene del mismo paciente; estos llegan a ser los de primera elección para la reconstrucción del reborde alveolar, por ser los únicos que presentan los tres mecanismos biológicos de osteogénesis, osteoinducción y osteoconducción que en conjunto brindan una óptima arquitectura al hueso receptor del injerto para su posterior rehabilitación. Por lo tanto y dependiendo de la zona a restaurar, el hueso autólogo puede ser obtenido de regiones específicas del cuerpo humano, entre los cuales destacan los injertos obtenidos de la zona intraoral como la sínfisis del mentón, la rama mandibular, y de la zona extraoral la cresta ilíaca ya que por ser huesos de tipo cortical-esponjoso ofrecen gran resistencia y composición. En este trabajo conoceremos todas las posibilidades terapéuticas del injerto autólogo empleando varios recursos basados en principios biológicos para llegar al protocolo quirúrgico que ofrezca mayores posibilidades de obtener una regeneración ósea estable, confiriendo mayores ventajas orientadas a la reconstrucción de reborde alveolar

Currently there are several bone grafts to reconstruct the bony structure of the ridge, which may be of autologous, homologous, heterologous and artificial origin, which have an important role depending on the tissue from which they are extracted, so that the more compatible and with better results will be the autologous graft, so named because it is obtained from the same patient; these become the first choice for the reconstruction of the alveolar ridge, since they are the only ones that present the three biological mechanisms of osteogenesis, osteoinduction and osteoconduction that together provide an optimal architecture to the graft receptor bone for later rehabilitation. Therefore, depending on the area to be restored, autologous bone can be obtained from specific regions of the human body, among which are the grafts obtained from the intraoral area such as the symphysis of the chin, the mandibular branch, and the extraoral zone the iliac crest since being cortical-spongy type bones offer great resistance and composition. In this work we will know all the therapeutic possibilities of the autologous graft using several resources based on biological principles to reach the surgical protocol that offers greater possibilities of obtaining a stable bone regeneration, conferring greater advantages oriented to the reconstruction of alveolar ridge
