Manejo de terapia pulpar, pulpotomía, pulpectomía, apicoformación

Abstract

Los dientes deciduos cumplen funciones muy importantes en la masticación, fonación, respiración y estética; por lo tanto cuando se encuentran afectados por lesiones cariosas es necesario darles tratamiento para conservarlas hasta que concluya la rizólisis, del mismo modo en los dientes permanentes jóvenes lo que se busca es preservarlos durante la etapa de rizogénesis. Sin embargo muchas piezas temporales y permanentes jóvenes son afectadas por factores bacterianos o traumatismos que comprometen su integridad y es indispensable que reciban tratamiento pulpar específico, ya que las piezas deciduas son diferentes a las piezas permanentes en cuanto a estructura, morfología y tamaño.

La Pulpa es el único tejido blando en el diente, cumple con múltiples funciones mientras no está lesionada, y de acuerdo al estado en el que se encuentra se pueden obtener diagnósticos como pulpa sana, pulpitis reversible, pulpitis irreversible y necrosis pulpar; estos diagnósticos nos llevan al tratamiento cuya finalidad es mantener la integridad de los dientes y tejidos de soporte para evitar una medida más drástica que sería la exodoncia. Los tratamientos pulpares no solo se realizan directamente sobre la pulpa también sobre la dentina desencadenando una respuesta de defensa, lo ideal es conservar a la pulpa vital pero cuando las condiciones ya no son tan favorables mantenerlo clínicamente funcional es la alternativa.

Pulp is the only soft tissue in the tooth, it fulfills multiple functions while it is not injured, and depending on the state in which it is found can be obtained diagnoses as healthy pulp, reversible pulpitis, irreversible pulpitis and pulp necrosis; these diagnoses lead us to the treatment whose purpose is to maintain the integrity of the teeth and support tissues to avoid a more drastic measure that would be the exodoncia. Pulp treatments are not only performed directly on the pulp also on the dentin triggering a defense response, the ideal is to conserve the vital pulp but when the conditions are no longer so favorable to keep it clinically functional is the alternative
